Can you give an example of a detailed QA report you have created in the past?

I have had to complete QA reports for my most recent internship position. As a Behavioral Health Service health coordinator, I was required to put together QA surveys and submit these reports. These were created to help the mental health providers learn more about what their patients and consumers needed. Effectively streamlining proper communication and understanding.
